EPSpeak for clinicians:patient-shared records

EPSpeak gives your patients a simple way to keep a daily record of how they are feeling and moving while taking antidepressant or antipsychotic medication, then share it with you by QR code at the appointment. It is free, needs no account, and is a self-tracking tool: it does not diagnose or make recommendations about treatment.

What your patient can bring to the appointment

Between appointments, patients build a patient-reported record on their own phone. When they choose to share it, you can see:

  • Daily check-in history: a short read-aloud affirmation of roughly 20 to 40 seconds, plus a brief scored wellness questionnaire
  • A voice-change score over time, which compares each recording with the patient's own earlier recordings, not with a population
  • Symptom notes in the patient's own words, including movement changes such as restlessness, tremor, stiffness or slowness
  • A medication log with doses and changes over time
  • Optional wearable data, such as steps, heart rate, sleep and exercise, if the patient has connected a Fitbit or Google Health

The aim is to give you weeks of context in the time it takes to scan a code, so more of a short consultation can go to decisions rather than recall.

How sharing works

Sharing is opt-in and controlled by the patient. They generate a QR code in the app and choose the clinician they share with. Nothing is sent to a clinician automatically. The patient can use a session at home before the appointment, or at the clinic during it.

Medication side effect profiles and their evidence base

For psychotropic medicines, EPSpeak shows three scores from 0 to 4, covering extrapyramidal symptoms, akathisia and anticholinergic effects. The scores are derived from published comparative tolerability research: Huhn et al. and Leucht et al. in The Lancet, the Maudsley Prescribing Guidelines in Psychiatry, and Chew et al. on anticholinergic activity.

The profiles describe a medicine across many people in the literature. They are not a measurement of the individual patient, not a prediction, and not a recommendation. They exist to support a shared conversation about trade-offs, including the anticholinergic burden when several medicines are combined.

EPSpeak is a self-monitoring and health information tool. It is not a medical device and is not included on the Australian Register of Therapeutic Goods. It does not diagnose, it does not name or rate any condition, and the voice-change score is not a clinical judgement. Interpretation, and every decision about treatment, stays with you and your patient.

Recommending EPSpeak to patients

Points patients often ask about:

  • Free on Google Play for Android and on the App Store for iPhone, with no ads and no in-app purchases.
  • No account and no login. Anonymous sign-in with a random ID, and no name, email address, phone number or precise location collected.
  • Works on a phone alone. A wearable is optional.
  • Recording happens only during the check-in. Voice recordings are deleted after 7 days, and raw movement sensor data never leaves the phone.
  • Developed by clinicians working in psychiatry, by MANOAI Pty Ltd in Perth.
